If you're using OS X, start with the built-in Speech options in System Preferences (From the Finder: Apple Menu > System Preferences > Speech > Text to Speech). From there, you can make selections for the voice, rate, and what will be spoken. You can choose text-to-speech for alerts, selected text and the clock.
You can also try VoiceOver, built into Mac OS X System Preferences (From the Finder: Apple Menu > System Preferences > Universal Access > Seeing > VoiceOver > Open VoiceOver Utility...). It lets you choose what will be spoken when your cursor is held over it, and even options like having characters or words spoken as you type.
Combined with other OS X Universal Access options, you can control it with the mouse or keyboard. For instance, it can also follow your text cursor as you move it with the keyboard arrows.
